VALIDATION EVIDENCE
What a Hospital MRF Validator Should Record as Evidence
A green result without provenance is difficult to defend or reproduce. An evidence-oriented MRF preflight should preserve what was tested, when it was tested, and which rules produced each finding.
Minimum technical record
The evidence record should uniquely identify the public artifact and the validation procedure.
- Public source URL
- Retrieval and validation timestamps
- File size and cryptographic hash
- Detected format and CMS template version
- Validator build and ruleset version
- Unabridged findings
Minimum operating record
Technical findings become useful when accountability and disposition are visible.
- Severity and plain-English explanation
- Named remediation owner
- Disposition and supporting note
- Retest result
- Final reviewer and review date
Avoid overstating the result
Describe the output as technical preflight evidence, not certification or a guarantee of regulatory compliance. A validator cannot independently establish the accuracy of upstream pricing and claims-derived inputs.
